Liability & Release Forms Required

Step 1-Please print these forms and fill them out.
Step 2-Please make sure to sign them along with your parent/guardian in front of a notary.
Step 3-Please make sure the notary signs and notarizes the forms.
Step 4-Turn these forms into your trip leader or Missions Director.
Missions Manual Required
Both parents and students need to read the Missions Manual in its entirety.
Requirements
Prerequisites
- Student must fill out the application for this trip in its entirety.
- Student must upload a front and back picture of their health insurance card.
- Student must upload a clear picture of an ID. Approved forms of ID: Passport Book/Card, Driver's License, Driver's Permit, or State ID. If you do NOT have any of these due to your age, you can upload a picture of your CAI/CAL ID. Please remember this will be the ID that we purchase plane tickets with so if you do NOT want to take your passport to Alaska then please submit a lesser form of identification.
Responsibilities
- Students will be responsible for attending all team meetings unless explicitly given permission by the team leader.
- Students will be responsible for raising all the funds required to go on this trip.
- Students are expected to uphold their grades at a satisfactory and passing standard.
- Students are also expected to behave satisfactorily.
Restrictions
- This trip is only open to Visual & Performing Art track students.
- You must be passing your classes to attend this trip.
- You must have satisfactory behavior to attend this trip.
- You must raise all the funds to attend this trip.
